• W2885175292 abstract "To clarify the indications for reconstructive surgery in patients with diaphragmatic hernia.Retrospective trial has included 36 patients with diaphragmatic hernia for the period 1963-2017. There were 23 (63.9%) women and 13 (36.1%) men. The majority of patients (83%) underwent surgery at able-bodied age (18-60 years). 27 (75%) patients had hernia of weak diaphragmatic zones, 9 (25%) - posttraumatic hernia. Diaphragm repair was performed with primary suture. In 2 cases of posttraumatic hernia mesh endoprosthesis was used.All patients were discharged. Postoperative complications arose in 4 (11.1%) patients, including 2 cases of mesh endoprosthesis deployment. Long-term outcome was followed-up in 15 patients from 6 months to 17 years. Recurrent hernia was absent in all cases.Primary suture is acceptable for diaphragmatic hernia repair. Alloplastic repair is indicated for large defect, when primary suture is impossible or risk of its failure is high.Цель исследования ― на основании изучения результатов хирургического лечения пациентов с грыжами слабых зон диафрагмы и посттравматическими диафрагмальными грыжами (ДГ) уточнить показания к различным вариантам операций. Материал и методы. В основу ретроспективного исследования, выполненного в период с 1963 по 2017 г., положен опыт хирургического лечения 36 больных ДГ. Женщин было 23 (63,9%), мужчин ― 13 (36,1%). Большинство больных (83%) оперировали в трудоспособном возрасте (от 18 до 60 лет). У 27 (75%) пациентов диагностировали грыжи слабых зон диафрагмы, у 9 (25%) ― посттравматические грыжи. Устранение травматического дефекта диафрагмы и анатомически слабого места выполняли первичным швом, за исключением 2 случаев посттравматических ДГ, когда использовался сетчатый эндопротез. Результаты. Все оперированные больные были выписаны. Послеоперационные осложнения возникли у 4 (11,1%) пациентов, в том числе у 2 человек, у которых был использован синтетический сетчатый эндопротез. Отдаленный результат прослежен у 15 пациентов в сроки от 6 мес до 17 лет. При контрольном рентгенологическом обследовании ни у одного пациента рецидива ДГ не было. Вывод. При устранении диафрагмального дефекта допустимо выполнение первичного шва. Показанием к аллопластическому замещению дефекта служат его большие размеры, когда первичный шов диафрагмы невозможен, или его состоятельность подвергается сомнению." @default.
