- W2885202410 abstract "Acute lymphoblastic leukaemia is the most frequent neoplasia in childhood with more than 85% of treated patients cured in high-income countries. 1 Tai EW Ward KC Bonaventure A Siegel DA Coleman MP Survival among children diagnosed with acute lymphoblastic leukemia in the United States, by race and age, 2001 to 2009: findings from the CONCORD-2 study. Cancer. 2017; 123: 5178-5189 Crossref PubMed Scopus (40) Google Scholar One of the key drugs in its treatment is L-asparaginase (ASNase). The activity of ASNase can be affected by different factors such as antibody production and different pharmacokinetics among formulations. Serum asparagine (Asn) deamination selectively eliminates leukaemia cells preserving normal cells, since normal cells have the ability to synthesise Asn intracellularly. 2 Mondelaers V Suciu S Moerlose B et al. Prolonged versus standard native E. coli asparaginase therapy in childhood acute lymphoblastic leukemia and non-Hodgkin lymphoma: Final results of the EORTC-CLG randomized phase III trial 58951. Crit Rev Oncol Hematol. 1998; 28: 97-113 Crossref PubMed Scopus (391) Google Scholar In order to ensure therapeutic benefit, it is necessary to measure ASNase activity, since different formulations and silent inactivation might exist (values below 0·1 IU/mL). 4 Boos J Werber G Schulze-Westhoff P et al. Anal Biochem. 2002; 309: 117-126 Crossref PubMed Scopus (66) Google Scholar described that different ASNase preparations could have different half-lives and different initial concentrations, probably because of distinct Escherichia coli strains. Thus, it is important to be aware of the drug preparation and the substantial variability in interpatient response. Monitoring asparaginase activityI believe the findings of Daiane Cecconello and colleagues1 do not provide sufficient evidence for decision making, given the space constraints of a Comment. Several methodological characteristics were not reported, and this could influence interpretation of the findings. Full-Text PDF Monitoring asparaginase activity – Authors' replyWe would like to thank Maria Inez Pordeus Gadelha for her questions and comments. Full-Text PDF Monitoring asparaginase activityThe findings of Daiane Cecconello and colleagues,1 published in The Lancet Oncology, show differences in the in-vivo activity of two dissimilar brands of Escherichia coli L-asparaginase (ASNase) used for the treatment of acute lymphoblastic leukaemia. Despite their similarities in sequence and in-vitro activity, a previous study showed that two ASNase products (aginasa [Medac, Kyowa, Japan] and leuginase [Beijing SL Pharmaceutical, Beijing, China]) had key differences, with leuginase having increased immunogenicity, product-related impurities, and the presence of β-lactamase.
