- W2888113102 abstract "目的：現在の我が国の終末期ケアでは，子が高齢な親の意思決定の中心的役割を担いうる現状にあり，高齢者が事前に終末期ケアの希望について子とコミュニケーションをとることは，高齢者本人と家族の終末期ケアに備えた準備行動といえる．ただし親子間コミュニケーションの実態について，子の立場から量的に検討した研究は限られている．そこで高齢の親を持つ子を対象に，終末期ケアに関する親子間のコミュニケーションの実態とその関連要因を検討することを目的とした．方法：65歳以上の実親を持つ1,590人を対象にインターネット調査を実施した．2015年10月の3日間で回答した1,010人を分析対象とした．調査内容は，対象者及びその親の属性，終末期ケアに関する親子間の会話の有無及び親との会話に対する子の態度であった．結果：回答者の平均年齢は48.5±8.8歳，男性が52.7%であった．親の平均年齢は77.1±7.9歳，男性が38.4%であった．これまでに親と終末期ケアに関する会話をしたことが「ある」は22.8%，「ない」は77.2%であった．終末期ケアに関する会話の有無を目的変数とした二項ロジスティック回帰分析を行った結果，息子－父より息子－母（オッズ比＜OR＞＝3.01）・娘－母（OR＝3.15），親の年齢が高い（OR＝1.03），親に大病経験あり（OR＝1.47），親と同居（OR＝2.08），子の会話に対する必要性の認識が高い（OR＝1.36），子の会話に対する情緒的回避が低い（OR＝0.68）と有意な関連がみられた．結論：高齢の親と子の終末期ケアに関する会話は乏しい現状が示された．親子の会話は，自然発生的に生起するのではなく，親側と子側の両方の要因を総合的に理解した上で，コミュニケーションを支援することが重要であるといえる．" @default.
