Chronic non-cancer pain remains a poorly treated problem in North America for which long-term opioid therapy has become a common treatment. However, opioids can lead to individual and population-level harms over both the short- and long-term. The dramatic increase in prescribing rates of opioids for chronic pain, driven by misleading claims about their effectiveness and low addictive potential, led to widespread problems with their use. There has been an alarming rise in the morbidity and mortality associated with prescription opioids, fentanyl analogues, and heroin. As these issues raise both clinical and population-level challenges, they also raise intersecting clinical and population-level ethical challenges. The objective of this chapter is to explore the ethical issues at the junction of chronic pain and substance use. We focus primarily on the ethical dimensions of the current opioid overdose crisis given the magnitude and complexity of the issue. Our analysis addresses the moral arguments advanced by members of the pharmaceutical industry that helped to accelerate the overdose epidemic; how recent constructions of addiction as a brain disease have yet to shape the discourse and practice of pain management; the role of distrust and stigmatized identities; and how widely adopted clinical and health policy tools in response to the overdose crisis, such as opioid treatment agreements and prescription drug monitoring programs, may further intensify stigma." @default.
