- W2890174768 abstract "Background: The weekend effect describes the observation that outcomes are worse on the weekend than weekday. The objectives of this study were to determine if weekend readmissions following pancreas resection differed from those on weekdays and to measure the impact on long-term outcomes. Methods: HCUP State Inpatient Database for Florida was used to identify patients that underwent pancreas resection for malignancy from 2010 to 2014. Primary exposure was the weekend effect defined by a weekend readmission within 30 days of discharge from surgery. Measured outcomes (for readmission encounters) included inpatient morbidity and mortality. One-year survival was calculated for patients with and without a weekend readmission. All analyses conducted with consideration of a hierarchical data structure (patients clustered within hospitals). Results: 2,443 patients met inclusion criteria and 134 patients had a weekend readmission. Patients with weekend readmissions had similar index lengths of stay (15.9 vs. 15.5 days, p = .73) and incidence of postoperative inpatient complications (22.4% vs. 22.3%, p = .98) compared to those with weekday readmissions. Weekend readmissions resulted in increased odds of inpatient mortality (aOR 2.7, 95% C.I.: 1.1 – 6.6) with similar odds of inpatient complications (aOR 1.6, 95% C.I.: 0.8–2.8). After adjustment, weekend readmission was associated with increased risk of 1-year mortality, HR 2.3 (95% C.I.: 1.1 – 4.4, see Figure). Conclusion: Weekend readmissions following pancreatic resection are associated with adverse outcomes. This is not explained by measured patient factors or clinical characteristics of the index hospital stay. Developing strategies to overcome the weekend effect can result in improved survival for patients undergoing pancreatic resection." @default.
