Underlying this question is whether pain should be considered a medical problem that requires a medical solution or should belong only to those suffering in pain, giving them the choice of how to manage it. The rectitude of seeking pain relief has long been a core religious issue. However, it became an ethical issue for clinicians when pain began to be considered a medical problem and not just something imposed on people by external or divine forces. Although the medicalization of pain was a gradual process occurring through the centuries, the process accelerated at the beginning of the 20th century in line with advances in medicine that rapidly changed the clinician's role from one of comforter, to one of curer. It is perhaps no coincidence that at about the same time, opioids – uniquely powerful analgesics, yet uniquely dangerous – were removed as an option for patients, with that choice placed firmly into the hands of clinicians. This happened because of the introduction of regulations: the moral burden was suddenly one for clinicians and not for patients. Many of the troubling decisions clinicians now face about pain treatment are centred on opioids and arise because the correct balance between benefits and harms for opioids is so hard to achieve. Is clinicians' duty simply to reduce the intensity of pain, or does harm reduction matter as well, and if so when?" @default.
