- W2890719168 abstract "Background: Laparoscopic liver resection (LLR) is increasingly utilized for patients with hepatocellular carcinoma (HCC). However, outcomes for patients requiring unplanned conversion to an open procedure during LLR are not fully described. Methods: Patients undergoing LLR for HCC from 2010 to 2014 were identified in the National Cancer Database. Patient and tumor characteristics, perioperative outcomes, and overall survival (OS) were compared between patients undergoing successful LLR and those requiring unplanned conversion. Results: LLR was attempted in 1108 patients with unplanned conversion occurring in 203 (18.3%). Unplanned conversion was associated with positive margins (11.8 vs 7.6%, p = 0.091) and prolonged LOS (median 7 vs 4 d, p < 0.001), along with greater 30-day (6.6 vs 3.4%, p = 0.067), and 90-day mortality (11.2 vs 5.7%, p = 0.015). Conversion was associated with significantly reduced OS compared to successful laparoscopic resection (median 39.8 vs 67.6 months, p < 0.001). After sub-stratifying based on the extent of resection, this difference remained significant only for major hepatectomy (22.8 months vs NR, p < 0.001), with the greatest rates of conversion occurring in those undergoing right or extended right hepatectomy.By multivariable analysis, tumor size ≥5 cm (OR 2.4), AJCC T 3/4 tumors (OR 1.7), major hepatectomy (OR 1.5) and advanced age (OR 2.6) were significantly associated with unplanned conversion. Conclusion: In this analysis of a large national database, unplanned conversion was associated with inferior perioperative outcomes and OS compared to successful LLR for patients with HCC. LLR should be considered cautiously in patients with larger, more advanced tumors likely to require major resection." @default.
