- W2890760511 abstract "Sir: As stated in “Ultrasound Technology in Targeting Reflux-Free Venules for Optimal Lymphaticovenular Anastomosis in the Treatment of Lymphedema”1 by Visconti et al., ultrasound can be a useful supplement to the vein visualizer. Dr. Hayashi and Dr. Visconti are the leading experts in using ultrasound to identify lymphatic vessels for lymphaticovenous anastomosis.1–4 Based on our previous publication,5 the advantages for using a vein visualizer to target a reflux-free vein were as follows: providing straightforward, easy-to-understand images; a short learning curve; portability; and a relatively lower cost compared with ultrasound. However, the shortcoming of the vein visualizer is the depth of superficial vein it can detect, which is limited by the thickness of subcutaneous fat, and it is suitable only for mild lymphedema patients with minimal or no dermal backflow. It is suitable for anatomical locations such as dorsal hand/foot, wrist, and elbow where the subcutaneous tissue is relatively thin. The unique property of ultrasound can overcome these limitations. However, the images obtained from ultrasound are not as intuitive as with the vein visualizer, especially when trying to identify a reflux-free vein. A longer learning curve is needed. However, in experienced hands, ultrasound might be the only device needed for preoperative lymphatic vessel and recipient vein identification.
