- W2890774928 abstract "There are a number of phrases used to describe the expected demographic shift where the number of adults over 65 is expected to double in the next 20 years. The Grey Tide, Silver Tsunami, an Age Wave, and the Grey Hoard are a few. It is reasonable to expect that with our aging population there will be a significant day-to-day impact on society and health care, and especially, primary care medicine. While all specialties will be tasked with older patients, I believe that primary care will be stressed with the management of the frail older adult and the associated family, social, community, and medical challenges that are inevitably part of geriatric care. This shift creates a very real health care and social Geriatric Imperative: not a descriptor but a call to action. The Geriatric Imperative requires that we support and create teams of skilled providers across specialties and professions to assess and manage older adults. The Geriatric Imperative requires time to assess, treat, explore family and social support, and coordinate elder care. The Geriatric Imperative requires federal, state, county, city, and community funding and support for community social support networks, senior centers, transportation, home health services. The Geriatric Imperative will require loosening of restrictive regulations that impair well-trained professionals from functioning in a team environment from the office, home, hospital, and nursing facility. The Geriatric Imperative will require multiple professionals (physicians, physical and occupational therapists, social workers, nursing, nurse practitioners, and physician assistants [PA]) to schedule time for counseling patient and family. The Geriatric Imperative will require employer accommodation for employees faced with the challenges of caring for children and aging parents. The Geriatric Imperative will demand new patient and family–centered innovations for place of care and bedside clinical diagnostics. And, finally, the Geriatric Imperative requires that we strongly advocate for payment mechanisms that recognize the demands of frail elder care and adequately compensate the provider for this time-intensive work. In this issue of Physician Assistant Clinics, we have created a primer on many of the social and clinical challenges we are and will be facing with an aging population. I am deeply indebted to the extraordinary physicians, pharmacists, and PAs who have contributed to this issue. I hope that this issue will serve as a resource for you as we all prepare for the challenges of caring and accommodating an aging population. I also hope that this issue will serve as a PA call to action and for us all to participate in the Geriatric Imperative." @default.
