- W2890780641 abstract "Background: Post-operative pancreatic fistula (POPF) remains a feared complication following robotic pancreato-duodenectomy (PD) There is currently no agreement on what type of pancreatic reconstruction should be carried out. We report the results of a prospective study comparing the rate of POPF between three different pancreatic reconstructions following robotic PD: Anterior pancreato-gastrostomy, posterior pancreato-gastrostomy and pancreato-jejunostomy. Methods: This study included 73 consecutive patients who underwent robot-assisted pancreato- duodenectomy at the Univeristy of Ilinois Health Science System between April 2007 and April 2017. Post-operative pancreatic fistulas were recorded according to the International Study Group of Pancretic Fistula classification . Post-operative fistulas frequencies were compared using the Chi-Square test. Results: 39 patients underwent anterior pancreato-gastrostomy, 23 pancreato-jejunostomy and 11 posterior pancreato-gastrostomy. The overall fistula rate (ALL SGPF types) was 6.8%. Of the 39 patients who received the anterior pancreato-gastrostomy we observed 1 type A POPF (2.5%). Of the 23 patients who received the posterior pancreato-jejunostomy 1 type B and 1 type C POPF were recorded (8.7%). Of the 11 patients who received a posterior pancreato-gastrostomy, 1 type A and 1 type C fistulas were observed (18%). Chi-square analysis failed to demonstrate a statistically significant difference of POPF occurrence amongst the 3 groups (P > 0.05) Conclusion: All three reconstructions resulted to be valid options for the reconstruction of the pancreatic stump following robotic pancreato-duodenectomy. Based on technical preference we favor either the anterior pancreato-gastrostomy or the pancreato-jejunostomy. Further studies are needed to clarify what type of reconstruction provides best results following PD." @default.
