- W2890789035 abstract "Background: Recurrence after curative-intent hepatectomy for hepatocellular carcinoma (HCC) is attributable to two unique patterns: recurrence from residual micrometastases and de novo recurrence due to carcinogenic potential in the underlying liver. The former is assumed to happen in the early post-operative period, suggested to be within 2-years. Within those early recurrence patients, some patients were noticed to recur very early, within 6 months, that we have termed very early recurrence (VER). This study aimed to investigate indicators of VER after hepatectomy. Methods: A retrospective analysis of patients who underwent hepatectomy for HCC and followed > 6 months between 2000 and 2015 at Cleveland Clinic and Johns Hopkins Hospitals. VER is the subset of patients that recurred ≤6 months from their initial surgery and Non-VER (nVER) are patients that recurred > 6 months from surgery. These were compared to patients that did not recur (NR). Results: In total, 346 patients were analyzed. The VER group had higher rates of EtOH use (46.2%), estimated blood loss (1692mL), non-R0 resection (22.7%), and vascular invasion (66.7%) compared to both the nVER (18.8%, 661.3 mL, 1.7% and 43.2%) and NR groups (24.8%, 520.2 mL, 7.2% and 25.0%), respectively (all p < 0.05). Preoperative AFP was higher in those that recurred, but not specific to the VER group (VER = 21.7, nVER = 16.0 and NR = 7.6). The preoperative predictors of VER based on linear regression models were EtOH use (HR = 3.2) and a T3 or T4 lesion (HR = 2.4). Lastly, VER patients had lower overall survival compared to both nVER and NR groups (20.4 v 41.6 v 36.0, p < 0.01). Conclusion: VER portends a poor prognosis, even compared to nVER, therefore it is an important group to investigate.
