• W2890840562 abstract "Abstract Curative-intent surgery for colon and rectal cancers is aimed at removing the tumor-bearing segment of the bowel with adequate margins and excising en bloc the mesentery containing the feeding vessels and regional lymph nodes. The extent of the resection depends on the location of the primary tumor, which determines lymphatic drainage. The advantages of laparoscopic surgery include less postoperative pain, earlier return of bowel function, shorter hospital stay, and a decreased rate of complications. Robotic platforms have further enhanced the visualization and dexterity available to the surgeon. Colon continuity can be reestablished using anastomotic techniques. Rectal cancer surgery is more challenging than colon cancer surgery and can lead to loss of bowel, urinary, and sexual function. The surgeon must take great care to avoid damaging the associated nerves. Total mesorectal excision proceeds along the areolar tissue plane situated between the mesorectal fascia and presacral fascia. The sphincter can be preserved by anastomosing the divided end of the descending or the sigmoid colon to the rectal stump. If the levator muscles or the sphincter is infiltrated by the tumor, however, an abdominoperineal excision is performed, entailing en bloc excision of the levator muscles and the anal canal, and the creation of a permanent end colostomy. Total mesorectal excision can be performed using a hybrid approach consisting of laparoscopic lymphovascular control and colon mobilization and open mesorectal dissection and anastomosis. For stage I rectal cancer, local excision can now be easily performed for tumors located in the mid and upper rectum." @default.
