- W2890933882 abstract "Partial anomalous pulmonary venous connection (PAPVC) is a rare congenital anomaly. When present, it most commonly involves the right-sided pulmonary veins (PV) draining into the superior vena cava, or directly into the right atrium, with left-sided PVs commonly draining into the vertical vein. PAPVCs are often asymptomatic, but if a significant left-to-right shunt is present and left untreated, right ventricular failure, irreversible pulmonary hypertension, or arrhythmias may develop. The principle behind surgical correction of PAPVC is the segregation of the systemic and pulmonary circulation, either by connecting the anomalous PV directly to the left atrium, or indirectly re-routing it through a baffle into the left atrium. Here, we present a rare case of a 65-year-old woman who presented with New York Heart Association Class III symptoms, gradually worsening over the past 10 years. Transthoracic echocardiogram showed a dilated coronary sinus, mildly dilated right ventricle, and a right ventricular systolic pressure of 39 mmHg. Cardiac magnetic resonance imaging and computed tomography cardiac angiogram confirmed anomalous drainage of the left superior and inferior PVs into the coronary sinus, with no atrial septal defect identified. The Qp:Qs ratio was 2:1. These findings were confirmed intraoperatively. The repair was performed by creating an atrial septal defect, then re-routing the coronary sinus, with a bovine pericardial patch as a baffle, into the left atrium. She had a successful outcome and was discharged home, in sinus rhythm, on postoperative day 7. At the 6-week follow-up, she reported her shortness of breath has resolved, and progress cardiac magnetic resonance imaging showed a patent baffle, Qp:Qs ratio of 1, and a reduction in the right ventricular volume. Although there are several case series on PAPVCs, a left-sided PV draining into the coronary sinus has rarely been reported. This case showed that while PAPVC to the coronary sinus is rare, the same surgical principles can be applied with successful outcome." @default.
