The treatment gap disproportionately burdens lower- and middle-income countries, and is exacerbated by concerns about restricting opioid access to combat the opioid epidemic in higher-income countries. This global burden of pain raises important social justice and ethical questions about what medical professionals and organizations, state actors, and non-government organizations must do to combat this global public health crisis. Interconnecting national and international issues at systemic and structural levels impose interwoven layers of disadvantages on pain sufferers, indicating that effective and equitable pain management demands the global community to go beyond a medical model in thinking about preventing and treating pain. By focusing on chronic pain, in this chapter we argue that it is an ethical imperative for all stakeholders to attend to the social justice dimensions of the burden of pain. We propose various ways that international stakeholders can incorporate pain research and management into international and national health agendas to foster understanding of the complexities of pain as well as promote safe, multimodal, multidisciplinary, and macro approaches to prevent and manage pain." @default.
