- W2891336043 abstract "Background: Bile duct injuries are rare but devastating complications. Thus far, Gadoxetate disodium (Eovist) enhanced MRI has not been described in the US as a useful imaging technique for visualizing the biliary system. We present 2 cases where MRI Eovist was used for detection of bile duct pathologies. Methods: RV is a 76 year old male who underwent a left liver lobectomy with a Roux-en-Y hepaticojejunostomy for an intrahepatic cholangiocarcinoma, final stage pT1N0M0R0. Patient presented to ER approximately 1 month after with right upper quadrant pain. CT showed a possible biloma and MRI Eovist was performed.CC is a 40 YO F who presented 6 days after laparoscopic cholecystectomy with bilious drainage from her right upper quadrant incision. MRI Eovist was performed. Results: RV MRI Eovist results - Bile leak at the hepaticojejunostomy. On 30 min delayed Eovist sequences, there is a jet of contrast at the inferior neck of the biloma. On the two-hour delayed sequences, there is increased contrast within the biloma with minimal contrast in the jejunum. CC MRI Eovist results - On the delayed phase, biliary tree is not dilated. CBD is 4 mm in diameter and can be followed down to the ampulla. No contrast leak is evident. Conclusion: Eovist enhanced MRI has been shown to increase liver to lesion contrast during the hepatocyte phase, improving lesion detection and characterization. Due to its excretion through the biliary system, MRI Eovist not only allows visualization of the intra- and extra-hepatic biliary system to ascertain, in the case of a suspected bile duct injury, whether a leak is present but also the location of the leak. In the case of RV, MRI Eovist showed preferential flow of the contrast into the biloma. In the case of CC, MRI Eovist confirmed no extravasation, saving the patient from an invasive surgical intervention." @default.
