- W2891796733 abstract "Background: Peripancreatic fluid collections occur in a small subset of severe acute pancreatitis patients. While many collections resolve spontaneously, some lead to persistent symptoms such as visceral compression, bleeding, or infection. Symptomatic fluid collections can be managed endoscopically, percutaneously or surgically. This case-report describes use of endoscopic ultrasound (EUS) to treat an infected peripancreatic pseudocyst not amenable to image-guided percutaneous drainage in a poor operative-risk patient. Furthermore, we review the literature for EUS-guided drainage through the fourth portion of the duodenum. Methods: A 59 year-old male known to have a long-standing history of complicated alcoholic pancreatitis presented with fever and abdominal pain. Abdominal computed tomography (CT) revealed a large mesenteric abscess in the left midabdomen, not accessible by image-guided percutaneous approach. Endoscopic ultrasound-guided drainage through the fourth portion of the duodenum was performed successfully without complication. Results: The patient's symptoms resolved following the procedure and he was discharged home on the second post-procedural day. At 8 week follow-up, the patient was back to his baseline functional status. Repeat CT scan showed complete resolution of the abscess. Conclusion: Patients with complicated pancreatitis require a multidisciplinary team-based approach for a tailored treatment plan. A traditional approach may not always be feasible in such cases. Based on this report, EUS-guided drainage through the fourth portion of the duodenum is a safe and effective option for symptomatic peri-pancreatic fluid collections. Consideration of this approach may obviate the need for a potentially morbid operation. To our knowledge, this is the second reported use of this technique in the literature." @default.
