- W2891823712 abstract "Background: Intraoperative blood salvage (IBS) is used to reduce allogeneic blood transfusions during liver transplantation (LT). In hepatocellular carcinoma (HCC) patients, however, IBS may lead to re-infusion and systemic dissemination of cancer cells. The objective of this meta-analysis was to compare recurrence-free survival (RFS) and overall survival (OS) in LT recipients with HCC using IBS vs. no IBS. Methods: Relevant electronic databases were searched to identify studies on the topic. Hazard ratios (HR) for RFS and OS were estimated from published time-to-event-analyses. In addition, we analyzed unpublished individual-level data from a sample of 202 patients. Generic inverse variance with random-effects model was used for meta-analysis. Results: Six retrospective studies representing 1121 HCC patients undergoing LT, including the unpublished sample, met inclusion criteria. There was no evidence of a significant association between use of IBS and RFS (HR 0.80, 95% CI 0.60–1.07, p 0.13) or OS (HR 1.33, 95% CI 0.88–2.01, p 0.17). Meta-analysis of separate estimates for 1-year survival, when most recurrences will be detected, again did not show evidence for increased risk associated with IBS use. We found little evidence for between-study heterogeneity. Conclusion: Currently available evidence suggests that use of IBS during LT in patients with HCC does not increase risk of cancer recurrence or mortality." @default.
