- W2894568920 abstract "Epidural analgesia (EA) is the golden standard for analgesic management in abdominal surgery. However, the golden standard in patients undergoing pancreatectomy has yet to be determined, demonstrated by the variety of reported use of EA (≈10%–84%). The aim of this study was to evaluate the pratice of analgesic management and outcomes after pancreatectomy in our tertiary referral center. All patients undergoing open pancreatectomy from June 2013 through December 2016 were reviewed. Patients were screened preoperative to determine eligibility for EA. The type of analgesia during surgery determined the group in which patients were included: EA or non-EA (N-EA). According to protocol EA is stopped on postoperative day (POD) 3. Primary outcome was mean painscore (11-point scale) per POD. In total, 163 (74.8%) patients with EA and 55 (25.2%) patients with N-EA (of which 48 (87.3%) patients received intravenous Morphine) were included. The groups were largely comparable regarding patient characteristics. Initial analgesia was stopped prematurely in 47 (28.8%) patients with EA and in 1 (2.1%) patient with intravenous Morphine. Patients with EA had significantly lower painscores on POD0-1, whereas they experienced higher painscores on POD3-4. Fluid balances were similar between the groups. The EA group received more catecholamines perioperative. Postoperative complications did not differ between the groups. The results of this study demonstrate EA is safe and reliable in selected patients after open pancreatectomy. Patients with EA experience significantly lower painscores in the first PODs, therefore extending the period of EA after POD3 could be considered." @default.
