- W2894770553 abstract "Introduction: Thermal ablation (TA) is widely used as a first-line treatment in patients (pts) with hepatocellular carcinoma (HCC). If percutaneous approach is unfeasible, laparoscopic thermal ablation (LTA) is a valid alternative. Aim of the study is to confirm the effectiveness of the LTA in challenging HCC nodules. Methods: 503 LTA were offered to patients with single nodule or up to three nodules smaller than 3 cm not suitable for surgical treatments or not suitable for percutaneous TA. Technical success was defined as total necrosis in the TA area at 1 month, technique effectiveness if this area was persistent at the follow-up. Local tumour progression (LTP) if reappearance of enhancing tissue occurred around TA area. We used: radiofrequency ablation (RFA) in 300 pts, microwave ablation (MWA) in 122, and RFA or MWS associated with intrahepatic vascular occlusion (IHVO) in 81. Results: Postoperative mortality was 0.4%, while major complications occurred in 2% of patients. Technical success was achieved in 467 patients (93%), with 100% after IHVO. Median follow-up was 30.4 months and 67% of patients developed intrahepatic recurrence. LTP occurred in 15% of patients and this rate was not influenced by problematic location of HCC. Technique effectiveness was primary influenced by HCC adjacent to vessels (67% vs 82%; p=0.01) and diameter >3 cm (69% vs 85%; p=0.001). Conclusions: This study represents the largest single-centre series on LTA for HCC on cirrhosis and shows that LTA is safe and achieves good results in patients with challenging HCC nodules not suitable for percutaneous approach." @default.
