- W2894792318 abstract "Epidemiological studies suggest the incidence of gallstones in sub Saharan Africa is low. Studies from 1980's suggested the rate of gallstones was increasing mainly due to dietary changes. Hypothesis - Gallstone disease is no longer uncommon in South Africa (SA) and is due to increasing BMI brought about by urbanization and dietary changes. Methods: Data was collected from a nationwide histology database of the South African National Health Laboratory Service for the period 2003 to 2015. 34294 cholecystectomy specimens were analysed and divided into periods 1 (2003–2008) and 2 (2009–2015). Data regarding trends in body mass index in the SA population were derived from 3 previously published studies. Urbanization ratios were obtained from Statistics South Africa. Results: In SA, the number of cholecystectomies increased by 37% in period 2 and was consistent among 7 of 8 provinces. Cois et. al found a significant year on year increase in BMI which correlates with our year on year increase in cholecystectomy rates. They showed lower obesity rates in rural communities. The Northern Cape was the only province with a decrease in cholecystectomy rates in period 2 (−28.3% (p = 0.0014)). This province also was the only one to demonstrate more urban to rural migration, with all other provinces showing the opposite. Conclusion: Increases in BMI in the SA population correlates with increases in cholecystectomy rates suggesting an increase in gallstone disease. Residing in a rural area appears to be protective from obesity with a likely decrease in gallstone disease and hence fewer cholecystectomies." @default.
