- W2894880851 abstract "Introduction: In Japan, resection volume for hepatectomy is mostly determined according to indocyanine green retention rate at 15 minutes (ICG15). However, it is unclarified how ICG15 is associated with postoperative complications. Method: Among 16780 patients registered for National Clinical Database in 2015, 11686 patients with ICG15 and information of hepatectomy were included in this study. There were 8156 male and 3530 female patients with a mean age of 68±10.6 years (HCC; 5543, Meta; 4001, ICC; 839, other tumors; 1303). The patients were divided into four groups; group A (ICG15 < 10%; n=5661), group B (10%≦ICG15< 20%; n=4381), group C (20%≦ICG15< 30%; n=1173) and group D (30%≦ICG 15; n=463). Hepatectomy procedures were classified as partial resection (n=3934), systematic subsegmentectomy (n=2055), monosectionectomy (n=2043), bisectionectomy (n=2993) and trisectionectomy (n=208). Surgical complications were classified using Clavien-Dindo classification (CD) and were evaluated according to ICG15. Results: Complications over CD-III increased significantly as the operation time became longer and the intraoperative bleeding amount increased (p< 0.001). As the ICG15 became bigger (group A to D), operative death in systematic subsegmentectomy, mono- and bisectionectomy, complications over CD-III in mono- and bisectionectomy, intractable ascites in all procedures, SSI in partial resection and trisectionectomy, and liver failure in mono- and bisectionectomy increased significantly. While bile leakage was not associated with ICG15. Conclusions: Operation time and intraoperative bleeding amount are significantly associated with postoperative severe complications. ICG15 is a good indicator for predicting operative death, complications over CD-III, intractable ascites, SSI and liver failure." @default.
