- W2894942227 abstract "SESSION TITLE: Outcomes and Management of Viral Infections in the ICU SESSION TYPE: Original Investigations PRESENTED ON: 10/08/2018 04:30 PM - 05:30 PM PURPOSE: ICU admission among patients with viral & atypical bacterial respiratory tract infections in Egypt is notproperly studied. The aim of this study to describe the epidemiological,comorbdities & outcome among these patietns. METHODS: this study was implemented at CUH; 5100 bed tertiary care hospital during the period of 2010-2014. Allhospitalized patients meeting the WHO case definition for severe respiratory tract infections &necessitating ICUadmission were included. Nasopharyngeal/oropharyngeal(NP/OP) swabs were collected and samples were testedusing reverse transcriptase-PCR for Influenza A&B, Human metapneumo virus(hMPV), Respiratory syncytialvirus(RSV), Parainfluenza virus (PIV 1,2,3,4),adeno virus, Boca virus ,Corona (NL64,NL63) ,Enterovirus, Rhinovirus(A,B) and atypical bacteria (Mycoplasma, Chlamydia, and Leigoneilla). RESULTS: 3207 patients enrolled, 33.5% (1075) patients were found to have a viral etiology& atypical bacteria.20.6% (219) of the cases admitted to ICU. 53.0% (116) were females. Mean age for ICU admitted cases and non ICUcases was 15.67(SD±22.720) & 3.14(SD±8.739) respectively with clinical significant difference (p-value<0.0001).within ICU admitted cases: 37.4%(82) RVS, 17.8%%(39) multiple viral infections,15% (33) influenza including 10cases pandemic HN1 2009, 9.5%(21) parainfluenza PIV, 8.6%(19) adeno virus, 6 %(13) rhino virus, 5%(11) hMPV,while no Boca or atypical bacteria were detected.Clinically significant difference was observed in patients with chronicobstructive pulmonary diseases (COPD),chronic renal diseases, pregnancy & smokers (p-value; <0.0001,<0.0001,0.037, <0.0001 respectively). 45.7% (100) have been assisted by mechanical ventilation. 69.5% (116) hadchest x-ray suggestive of pneumonia with clinically significant difference (p-value <0.0001) . 55.8 %( 121) discharged,33.2 %( 72) transferred while 11.1% (24) Died with clinically significant difference (p-value < 0.000). CONCLUSIONS: viral infections are common causes of ICU admission and responsible for 11 % mortality in ICUadmitted cases.RSV is the commonest viral infection among ICU admitted cases. COPD, smoking, pregnancy, andchronic renal problems are the common comorbdities associated with ICU admissions due to viral infections. CLINICAL IMPLICATIONS: viral infections are a common causes of ICU admission and responsible for 11 %mortality in ICU admitted cases.
