- W2895032017 abstract "Introduction: Pancreatic neuroendocrine tumour (PNET) are rare tumours and well differentiated PNET are associated with relatively indolent physiological behavior. However, G1-2 PNET correlate with a non-negligible risk of recurrence and the most important risk factors can only be detected postoperatively. The aim of this study is to explore which preoperative factors are associated with poorer disease-free survival (DFS). Methods: This is a retrospective multiinstitutional study. Patients submitted to pancreatic resection from 6 Spanish centers were reviewed. Only patients with well differentiated tumours (G1–G2) were included. Demographic and clinopathological variables were analysed. Results: Data from 126 patients were reviewed. Median age was 59 (25–84) years . Ninety-six patients (76.2%) had a non-functioning tumour and G2 was observed in 12 (9.5%) patients. Lymphadenectomy was performed in 78 cases (61.9%). Recurrence occurred in 18 (14.3) patients. Median DFS was 4.5 years. At univariate analysis, male gender, tumour size and vascular involvement were associated with shorter DFS. At multivariate analysis, preoperative tumour size was the only factor associated with poorer DFS (p = 0.003, OR 1.2). Optimal cut- off value of 20 mm was determined using ROC curve analysis. When matched with postoperative factors associated to poorer DFS (grade, perineural invasion and lymphnode metastasis), preoperative tumour size >20 mm was still statistically significant (p = 0.04, OR 8). Conclusion: Tumours size >20 mm on preoperative imaging was the only preoperative factor associated with poorer DFS in G1 and G2 pancreatic neuroendocrine tumours and therefore it should be considered an important information before planning surgical treatment." @default.
