- W2895838023 abstract "Alzheimer's disease (AD) is the most common form of late life dementia. Up to 20% of patients with AD experience hallucinations during their illness. Visual hallucinations (VH) are known to be more common in Lewy Body dementia (DLB). In autopsy studies up to 60% of patients with AD had concomitant Lewy body pathology. Several studies have found that visual hallucinations are more likely in those with AD and Lewy body pathology. Decreased perfusion of the occipital lobe has been shown in those with VH but as Lewy bodies are sparse in this region they cannot explain the hallucinations. Post-mortem studies have suggested decreased cholinergic activity in the visual cortex in DLB and decreased perfusion in the occipital cortex in DLB, which appeared to be a result of decreased microvessel density secondary to decreased VEGF. Recent work has suggested that decreased cholinergic input may cause the decreased perfusion. We obtained tissue from a cohort matched for age, gender and post-mortem interval comprising 23 individuals with AD who had experienced visual hallucinations, 19 individuals with AD without hallucinations, 19 individuals with DLB and 36 controls. We studied BA18 (visual area V2) and BA19 (visual area V3) Vascular endothelial growth factor (VEGF, a marker of tissue hypoxia), myelin-associated glycoprotein:proteolipid protein 1 (MAG:PLP1) ratio (a measure of tissue oxygenation relative to metabolic demand) and α-synuclein were quantified by ELISA, Von Willebrand factor (a marker of endothelial cell content) by dot blot and α-synuclein also by immunohistochemistry, as in our previous studies. Choline acetyl-transferase activity was quantified by chemiluminescence. Routine neuropathological data were also available. Data was analysed using parametric statistical tests wherever possible, with the MAG:PLP ratio as the primary outcome. The MAG:PLP ratio was reduced in controls compared to the dementia groups in BA18 (p=0.034) and unchanged in BA19. There was no strong evidence of a between group difference in ChAT activity, VEGF or vWF." @default.
