- W2895840508 abstract "Assessment of the patient experience with outpatient care is typically performed using patient satisfaction surveys that contain multiple domains of experience as well as the respondents overall satisfaction. These different domains interact and influence each other, however, and it is possible that some are more important than others for overall satisfaction. Knowing these relationships will help direct targeted interventions. This study aimed to assess the relationships among various patient experience domains with overall satisfaction at a single institution and further explored which domains of patient satisfaction had the greatest contribution to overall satisfaction. From 2/2016 to 12/2017, we collected 583 surveys of patient satisfaction originating from a single radiation oncology department obtained from a CMS-approved vendor. The surveys used 20 questions to assess 6 domains: Registration, Facility, Treatment, Physicians (added locally), Personal Issues and Overall Assessment. The respondents evaluated their satisfaction using a Likert scale with following designations: 1=very poor; 2=poor; 3=fair; 4=good; and 5=very good. Several conceptual structural models were proposed and tested. A total of 424 surveys had an answer for each question and were therefore considered valid. Most respondents were women (61%), were seen in follow-up (66%), had a primary breast cancer (40%) and were treated curatively (71%). The Cronbach’s alpha coefficients were all satisfactory for all multi-item scales from 0.78 to 0.94. The Root Mean Square Error of Approximation fit indices for the final model was 0.001 with a Comparative Fit Index of 1.000, and Tucker-Lewis Index of 1.000. Facility, Physician, and Personal Issues were mediated by Treatment and Registration which contributed 85% to the variance of Overall Assessment. Facility contributed 78% to the variance of Registration. Physician and Personal Issues contributed 97% to the variance on Treatment. Unit increases in Registration and Treatment increased the Overall Assessment by 0.23 and 0.85. Unit increases in Facility improved Registration score by 0.86. Unit increases in Physician and Personal Issues increased Treatment score correspondingly by 0.52 and 0.50. The clinical domain with the greatest impact on Overall Assessment was Treatment. This domain was assessed with five questions about the staff: courtesy, explanations, skill, concern for comfort and concern for questions. Efforts to improve these five assessments have the greatest likelihood to impact the Overall Assessment. Improvements in Registration will also improve Overall Assessment." @default.
