- W2895842533 abstract "Olfactory dysfunction (OD) is common but largely ignored in Alzheimer disease(AD. The relationship between OD, clinical features and its neuroimaging, including DTI and fMRI changes remains unclear. Olfactory function was screened by Hyposmia Rating Scale (HRS) and evaluated by Sniffin’ Sticks test, from which TDI score was gotten. TDI consisted of threshold (THR), discrimination (DIS), identification (ID) and classified patients into AD with and with no OD (AD-OD and AD-NOD) groups. Cognitive and neuropsychological symptoms and activities of daily living (ADL) were evaluated by a body of rating scales. DTI and resting-state fMRI were performed. FA values and ALFF and ReHo were statistically analyzed. Total 39.2% of AD patients were screened to have OD by HRS and 45.4% of them were demonstrated to have OD by Sniffin’ Sticks test. In AD-OD group, the scores of TDI, THR, DIS, ID and HRS were all significantly lower than that in AD-NOD group (P<0.01). In OD group, the scores of Mini-Mental State Examination (MMSE), Auditory Verbal Learning Test (AVLT)-delayed memory, total AVLT, Complex Figure Test (CFT)-delayed, Symbol Digit Modalities Test (SDMT) and CFT-imitated were significantly lower and the time of Trail Making Test (TMT)-A, the score of Modified Apathy Estimate Scale (MAES), ADL were significantly higher than that in AD-OD group (P<0.05). AD-OD group exhibited significantly decreased FA value in the right cerebral peduncle and left tapetum compared with the AD-NOD group (P<0.05). FA value in the left tapetum was positively correlated with ID score. AD-OD patients exhibited decreased ALFF value in both superior temporal gyrus, left limbic lobe and cingulate gyrus and decreased ReHo value in right superior temporal gyrus. OD has more serious damages in multiple cognitive domains and serious impact on ADL of AD patients. The white matter integrity in the right cerebral peduncle and left tapetum of AD-OD patients is severely damaged, which was associated with OD. AD-OD may have function inhibition in both superior temporal gyrus, left limbic lobe, cingulate gyrus and abnormal brain activity in the right superior temporal gyrus." @default.
