- W2895893268 abstract "To compare an in-house multi-atlas contouring system to a newly released commercial implementation as part of the commercial system commissioning for clinical use. We have been using an in-house fully automated multi-atlas contouring system for 7 years. This has significantly outperformed many commercially available systems in saving time for dosimetrists in contouring and improving contouring consistency for treatment planning. A commercial implementation of multi-atlas segmentation has recently be released as part of a commercial treatment planning system. This commercial solution has a similar, but not identical, approach to our in-house solution. We compared the results of the two systems for contouring normal tissues from simulation CT images in the head and neck on 12 patients using leave-one-out validation. Manual contours on these 12 patients were checked for consistency and approved from clinical use by a subspecialized radiation oncologist. The auto-segmented contours generated from both systems were compared with the manual contours using Dice, mean surface distance (MSD), and Hausdorff distance (HD) for the following structures: brain, brainstem, spinal cord, parotids, mandible, cochleae, eyes, and lungs. We found that the in-house and commercial systems performed similarly in terms of segmentation accuracy measured by Dice, MSD, and HD (Table 1). The in-house system performed slightly better in contouring brainstem, parotids, and eyes, while the commercial system performed slightly better in contouring cochleae. The time in completing segmentation for one patient is about 9 minutes and 16 minutes for in-house and commercial systems, respectively. This evaluation suggest that this commercial system may be used in clinic for autocontouring head and neck normal tissues, though further examination of the auto-segmented contours is needed when commissioning the tool for clinical use. Multi-atlas segmentation as a well-developed technique is gradually adopted for clinical use.
