• W2895900803 abstract "Single voxel 1H MR spectroscopy (MRS) measurements from the posterior cingulate gyrus (PCC) have identified neurochemical abnormalities in both asymptomatic and symptomatic carriers of microtubule-associated protein tau (MAPT) mutation. The objective of this study was to determine the differences in diagnostic performance between MRS measurements obtained from the PCC and from the frontal lobe in distinguishing asymptomatic and symptomatic MAPT carriers from healthy controls. Twenty-one patients with MAPT mutations from six families (5 with N279K, 5 with V337M, 4 with IVS9-10G>T, 4 with P301L, 1 with S305N and 2 with R406W mutations) underwent single voxel MRS from both PCC and frontal lobe at 3T. Asymptomatic patients (n=10) had median clinical dementia rating sum of boxes (CDR-SOB) score of zero and symptomatic patients (n=11) had median CDR-SOB of 2. Age and sex matched controls (n=26) were recruited. Area under the receiver-operating characteristic curve (AUROC) analysis was used to assess diagnostic accuracy of neurochemical measurements for distinguishing asymptomatic and symptomatic MAPT carriers from controls. There was no significant difference in gender and education between groups, while asymptomatic group (35.3±10.3 years) was significantly younger than symptomatic group (51.8±8.9 years, p=0.0028) and healthy controls (49.6±14.3 years, p=0.0116). As expected, symptomatic group had significantly lower MoCA scores (19.8±10.4) than asymptomatic group and healthy controls (p=0.0338, p=0.0415, respectively). The ratio of the neuronal metabolite N-acetylaspartate to glial metabolite myo-inositol (NAA/mI) from frontal lobe showed the best diagnostic accuracy in distinguishing symptomatic patients from controls (sensitivity 72.7%, specificity 92.3%, AUROC 0.836). The ratio of the glial metabolite myo-inositol to the reference metabolite creatine (mI/Cr) from the PCC showed the best diagnostic accuracy in distinguishing asymptomatic patients from controls (sensitivity 60.0%, specificity 61.54%, AUROC 0.623). However, the AUROC for each of the measurements was not different between PCC and frontal MRS voxels (p>0.05)." @default.
