- W2895917214 abstract "MMSE and Hasegawa are widely used across Japan in real world clinical practice to assess the severity of CI. Whilst it is known that there is a substantial level of burden on the caregiver of patients with CI; there is a need to understand what causes caregivers the most strain and how disease severity impacts this. The objective of this analysis was to investigate the relationship between MMSE and caregiver strain of patients with cognitive impairment (CI). Data were drawn from the Adelphi Dementia Disease Specific Programme, a global (Australia, Canada, France, Germany, Italy, Japan, Spain, U.K., U.S.) cross-sectional study of consulting patients with CI. Patients in Japan with physician-reported scores for their most recent MMSE, or Hasegawa, were identified. Multiple linear and negative binomial regressions modeled the relationship between MMSE and caregiver-reported EQ-5D, ZBI and physician-reported professional caregiver hours, respectively, with MMSE as independent variable. Missing MMSE scores were imputed from Hasegawa scores, using multiple imputation. Regressions adjusted for age, sex, BMI, comorbid conditions, concomitant medications, behavioural symptoms and retirement status. Analysis on EQ-5D and ZBI was conducted on 98 caregivers. Decreasing MMSE was significantly associated with increased ZBI overall strain (e.g. +0.21 from MMSE 27-26, +0.57 from MMSE 21-20, p=0.038); personal strain (e.g. +0.26 from MMSE 27-26, +0.42 from MMSE 21-20, p=0.041); and role strain (e.g. +0.18 from MMSE 21-20, +0.49 from MMSE 16-15, p=0.031). MMSE was not significantly associated with EQ-5D (p=0.396) or ZBI guilt (p=0.301). Analysis on professional caregiver hours was conducted on 631 patients, and decreasing MMSE was significantly associated with increased requirement (e.g. +0.61 hours from MMSE 27-26, +1.08 hours from MMSE 21-20, p<0.001). There is a strong association between MMSE and overall level of strain, role strain and personal strain felt by the caregiver as measured by ZBI. Interventions targeting patients early could help delay or relieve the strain put on caregivers as severity increases." @default.
