- W2895917807 abstract "Longitudinal patient data is rare, so diagnoses are usually done in reference to group averages. With Alzheimer's Disease, these group averages must reflect the changes of normal aging yet exclude the impact of preclinical Alzheimer's Disease, which is prevalent in older populations. Here we have compiled a Super-Normal Cohort (SNC) that establishes the group-average volumetric changes in the aging brain. In addition, we have developed a tool called the ILP (Individual Longitudinal Participant) report that uses open source software to plot individual patients’ MR volumetrics and cortical thicknesses onto the calculated normal aging lines. When diagnosing Alzheimer's, the primary volumetrics of interest are hippocampi volumes and lateral ventricle volumes. The SNC contains subjects that have undergone a structural MRI, were CDR=0 at baseline, and remained non-demented at least 3 years after MRI collection. The SNC excludes individuals with Alzheimer pathology as defined by β-Amyloid PIB PET mean cortical binding potential >0.18, CSF Aβ1–42 values <459 pg/mL, t-tau >339 pg/mL, and/or p-tau >67 pg/mL. The open source software Freesurfer and R were used for all data processing and analysis. Volumes were normalized to intracranial volume, and a weighted LOESS regression was used to determine changes with age. The ILP report is an R-based tool used clinically to plot an individual's MR data on top of this regression line and the first two standard deviations. The SNC was established with 194 subjects ranging in age from 43-90. When corrected for total intracranial volume, combined left and right hippocampi volume declined on average 50.4 mm3 per year.
