- W2896029667 abstract "A primigravida was referred to our centre for a fetal peri-orbital tumour diagnosed at third trimester scan. Ultrasound examination at 32 gestational weeks (GW) showed a hypo-echoic peri-orbital mass of 25x8 mm extending to the inferior, lateral and external aspects of the right orbit, presenting both central and peripheral vascularisation. Antenatal magnetic resonance imaging (MRI) at 33 GW showed iso- to hyper-intense areas inside the lesion with no anomalies of T1 and T2 signal and no optic nerve involvement. Serial US showed stability of the lesion during pregnancy. A healthy female was spontaneously delivered at 40 GW. Postnatal MRI confirmed the presence of the orbital mass associated with multiple intra-parenchymal cerebral lesions, located in the white substance especially in the junction zone. Biopsy of the orbital lesion revealed the presence of astrocytes and glial tissue within the microvascular stroma, positive for PS100 and GFAP immunohistochemical markers. Diagnosis of glial heterotopia was retained with associated glial intracranial-extra cerebral lesions. Mass spontaneously reduced in size, cerebral lesions disappeared, and neurological and ophthalmological follow-up was normal at 6 months of life. Glial heterotopy is defined as the presence of normal neural tissue outside the cranial cavity or spinal cord, without continuity with the brain and the meninges. A wide variety of lesions may develop around the orbit including benign or malign tumours, vascular and inflammatory lesions. Each lesion should be clearly described in term of localisation, size and extent of disease, echogenicity/intensity and vascularisation in order to orient diagnosis and antenatal counselling regarding postnatal treatment. Malignancy is suspected in case of irregular, rapidly growing masses deforming the orbit or/and the face. For a complete radiographic assessment of the lesion, a postnatal MRI study and a complementary CT scan are recommended.
