- W2896048907 abstract "To assess the correlation between the pelvic floor dimensions and the engagement of the fetal head, and the correlation between both parameters and the duration of 2nd stage of labour. We enrolled a group of nulliparous women at term before the onset of labour. We acquired transperineal ultrasound images and measured the anteroposterior diameter (APD) of pelvic hiatus and the angle of progression (AoP) of the fetal head at rest, during pelvic floor muscle contraction (PFMC) and during Valsalva. We assessed the correlation between APD and AoP values. We also assessed the correlation between AoP and APD and duration of 2nd stage of labour. Overall, 222 women were included in the study. Among these, 164/222 (73.9%) delivered vaginally, while 58/222 (26.1%) underwent Caesarean section. We found a statistically significant direct correlation between APD and AoP during PFMC (rs 0.145, p=0.031) and during Valsalva manoeuvre (rs 0.143, p=0.034). Furthermore, we found a significant inverse correlation between APD and 2nd stage duration at rest (rs -0.233, P=0.002); during PFMC (rs -0.208, P=0.006) and during Valsalva (rs -0.266, P= 0.003). Lastly, 2nd stage duration inversely correlated with AoP during PFMC (rs -0.162 p value = 0.034), and at Valsalva (rs -0.167 p value = 0.029). In nulliparous women at term, larger pelvic floor dimensions are associated with a deeper engagement of the fetal head. Larger diameters of the pelvic floor hiatus and wider AoP are associated with shorter 2nd stage of labour." @default.
