- W2896088975 abstract "Routinely, the venous access point in endovenous treatment of venous ulcers is above the ulcer, around the knee area. We present our experience using inframalleolar access. During a period of 2 years, 20 patients with active chronic venous ulcers were treated using inframalleolar access. The type of access, method of treatment, vein accessed, uppermost treatment point, and adjunct procedures were recorded. Also recorded were complications and the time to healing of the ulcer. Images of the ulcers at 1 week, 4 weeks, and 12 weeks were obtained. A total of 24 veins were treated in 20 patients. Of those, 17 were great saphenous veins and 7 were small saphenous veins. In four patients, both veins were treated. The uppermost part of the great saphenous vein treated was the saphenofemoral junction in 8 patients and the knee area in 11 patients. The uppermost small saphenous vein area treated was the midcalf in five patients and the vein of Giacomini above the knee in two patients. The treatment was with mechanochemical ablation in all patients, with the addition of foam in 40%. No complications were seen. There was no instance of deep venous thrombosis; 50% of patients received a single dose of anticoagulation during the procedure. At follow-up ultrasound examination, all treated veins were closed. All ulcers but one were healed at 3 months. No recurrence was seen. Inframalleolar venous access is feasible and safe. It appears to enhance venous ulcer healing." @default.
