- W2896091972 abstract "High blood pressure (BP) has been linked to cerebral small vessel disease as manifested by white matter hyperintensities (WMH) on brain MRI in late-life. Endothelial function, as measured by cerebral vasoreactivity (VR), has also been associated with larger WMH volumes in late-life. However, the significance of these relationships have not been explored in midlife nor across race. We included 85 subjects from the Coronary Artery Risk Development in Young Adults (CARDIA), a community based cohort enrolled in 1985 and followed for 30 years. Inclusion criteria composed of current presence of one or more vascular risk factors. Systolic blood pressure (SBP) was measured in 2015 (year 30). VR was measured as the slope of the relationship between beat-to-beat blood flow velocity in the middle cerebral artery in response to changes in end-tidal CO2. Participants also completed a 3T brain MRI for measures of small vessel disease; specifically: white and gray matter volumes as a percent of intracranial volume; WMH as a percent of total white matter volume, and DTI measures of fractional anisotropy (FA) and mean diffusivity (MD) in normal white matter and WMH. All analyses were adjusted for age and sex. The mean age (SD) was 56 (3.8) years old (44% female, 53% blacks). There was no relationship between both SBP and MRI measures and between SBP and VR (Table 1). However, blacks had significantly higher blood pressure (126mHg (16) vs117mmHg (13), p=0.006) but a higher VR (EtCO2 covariance, % change (SD)) (0.6 (0.4) vs. 0.8 (0.5), p=0.024), than the whites from baseline. DTI measures were significantly worse in normal appearing white matter (NAWM), but not in WMH (NAWM vs WMH)(FA p=<0.001, p=0.872; MD p=0.005, p=0.564). Our findings suggest that the effect of BP exposure on cerebral small vessel disease and its relationship to endothelial function (VR), as demonstrated by microstructural changes, is not fully developed in mid-life. Coupled with the race difference in our findings, there is potential to better understand the effect of genetics or varying blood pressure exposure in affecting cerebral small vessel disease differently in adults through mid-life." @default.
