- W2896411675 abstract "Cardiovascular health appears related to late life cognition. In particular, the presence of cardiovascular risk factors in midlife is frequently associated with late life cognition and dementia. While the QT interval has been associated with major cardiovascular events like myocardial infarction and stroke, it has less frequently been examined in relation to cognitive outcomes. Thus, we sought to evaluate whether the onset of prolonged, rate-corrected QT interval (QTc) in midlife is associated with longitudinal cognitive performance approximately 25 years later. We used data from the Honolulu-Asia Aging Study, a cohort of Japanese American men originally recruited at ages 46 - 68 in 1965-1968 as part of the Honolulu Heart Program. Midlife electrocardiography measures including QT interval and heart rate were assessed during 3 study visits (Visits 1 to 3) between 1965 and 1974. We defined incident prolonged QTc as a QTc interval in the upper quartile (>406 milliseconds). Persons lacking prolonged QTc at baseline were followed to see if they developed incident prolonged QTc at the second or third follow-up visits. Cognitive performance was assessed using the Cognitive Abilities Screening Instrument (CASI) during 4 study visits (Visit 4 to 7) between 1991 and 2000 2000, and scored using item response theory, so that the mean score atvisit 4 was 0.4 (SD 0.9). Primary models used inverse probability weighting to account for confounding and potential for bias due to informative attrition. Of the 2,511 HAAS participants who contributed cognitive data to our analyses, 43% had incident prolonged QTc at either Visit 2 or Visit 3. There was no association of incident prolonged QTc with either baseline CASI scores (estimated difference = 0.09; 95% confidence interval [CI]: -0.23 to 0.40; P = 0.59), or change in late-life CASI scores over time (estimated difference = -0.003 per year; 95%CI: -0.014 to 0.008; P = 0.58) in analyses weighted to account for confounding and study attrition. Findings were materially unchanged in additional sensitivity analyses.
