- W2896513119 abstract "Alzheimer's disease (AD) and Lewy Body dementia (LBD) are two common forms of dementia. There are still controversies regarding whether LBD patients have a worse clinical course when compared with AD patients. All biomarkers supported AD and LBD patients presented to the memory clinic of Queen Mary Hospital between 1st January 2008 and 30th December, 2016 were retrospectively reviewed. The diagnoses of AD and LBD should be supported by both clinical diagnostic criteria and biomarkers (including cerebrospinal fluid biomarkers or functional imaging ± pathological biomarkers). LBD patients included dementia with Lewy Bodies (DLB) and Parkinson's disease dementia (PDD). Baseline demographics, Mini-Mental State Examination (MMSE) scores, Clinical Dementia Rating (CDR) and presenting clinical symptoms were traced. Endpoints including fall, dysphagia, pneumonia, pressure injuries, institutionalization and mortality were traced and time to these events were recorded. The initial clinical presenting features and long term outcomes were compared. There were 31 AD patients and 25 LBD patients recruited (18 with DLB and 7 with PDD). Upon presentation, two groups of patients had comparable baseline demographics, MMSE and CDR. LBD patients suffered from more hallucination and anxiety symptoms according to NPI subscores (both median (IQR) 1 (4) vs. 0 (0), p=0.01) than AD patients. There was no difference in disease duration. Proportionately more DLB patients suffered from falls (72 vs. 22.6%, p<0.001), dysphagia (52 vs. 12.9%, p=0.003), pneumonia (40 vs. 16.1%, p=0.045), and mortality (56 vs. 25.8%, p=0.02) than AD patients. There were no differences in the time to these events between two groups. Upon clinical presentation, LBD patients presented more commonly with hallucination and anxiety. In long term, LBD patients suffered from more falls, dysphagia, pneumonia, and mortality than AD patients although the time to these events showed no difference." @default.
