- W2896533592 abstract "Epidemiological studies have focused on the effects of specific sleep disorders on cognition in the elderly. However, sleep health can be measured across multiple dimensions, and the overall sleep profile may be more consistently and strongly related to cognition. A simple, aggregate measure of self-reported “sleep health” has been developed and shown to be closely related to health outcomes. However, almost nothing is known about its association with cognitive outcomes in older adults. We examined 2828 older men (aged 76±5 years) without significant cognitive impairment at baseline. Self-reported sleep over the past year was categorized as “good” or “poor” based on five sleep dimensions: subjective sleep satisfaction, daytime sleepiness, mid-sleep time, sleep onset latency, and sleep duration. An aggregate measure of sleep health was derived by summing the number of “poor” dimensions, with total score ranging from 0 to 5 and higher scores indicating poorer sleep health. Random-effects models were used to examine the association between sleep health and its dimensions and change in cognitive function from baseline over 10-12 years (10.8 ± 0.5) with 4 follow-up visits, as assessed by a test of global cognition, Modified Mini-Mental State Examination (3MS), and a test of executive function, Trails B. After adjustment for age, race, clinic, education, Body Mass Index, depressive symptoms, smoking, alcohol use, physical activity, benzodiazepine or sleep medication use and comorbidities, 3MS score decreased by 3.9 points (p<0.01) and Trails B score increased by 43.5 seconds (p<0.01) over 10 years, for those with at least 1 poor sleep health dimension compared to those with none (2.9-point decrease and 36.8 sec increase, respectively). The adjusted change in Trails B scores were 36.7, 42.7, and 46.7 sec (p for trend<0.01), and in 3MS scores 2.9, 4.0 and 3.5 points (p for trend=0.05), for those with none, 1-2, and 3-5 “poor” sleep health dimensions, respectively. Among older men without dementia, a multidimensional measure of sleep health was significantly associated with long-term decline in both global cognition and executive function. Sleep health as a simple, self-reported measure might be a valuable risk marker of future cognitive decline." @default.
