- W2896568493 abstract "Preventable medication errors affect more than 7 million patients, contribute to 7000 deaths and cost nearly $21 billion every year in the USA. In particular, this impacts on older people with dementia living at home with multi-morbidity and polypharmacy, their carers and practitioners, including GP's, nurses and pharmacists. This NIHR funded study uses realist methodology to understand how medication management works for those involved and under what circumstances. It integrates evidence from the literature with narrative accounts of people's experiences. It will inform the development of a framework to improve outcomes and support practice. MEMORABLE uses realist methodology in 3 integrated work packages: Work Package 1 (WP1): literature review: focusing on the research question; developing the scope of candidate programme theories and search strategy; selecting and appraising articles; and analysing/synthesising key articles (n=24) to develop and refine candidate programme theories and associated context, mechanism, outcome configurations. Evidence-based candidate programme theories about how medication management works are developed. Work Package 2 (WP2): 2-stage collaborative data gathering, analysis and review process involving older people (n=20), carers (n=10) and practitioners (n=20): initial participant interviewing for experiential, causal accounts of how medication management works; and following analysis, including whether this data confirms, refines or refutes the candidate programme theories, reviewing with participants what is now known about how medication management might work better. Evidence-based candidate programme theories are revised to reflect people's experiences. Work Package 3 (WP3): using the new knowledge to develop a framework to improve outcomes, including identifying the key mechanisms and their contextual triggers that appear most likely to have a beneficial effect and ways these can be supported in practice. MEMORABLE is work in progress, running from May 2017 to December 2018. At January 2018, 3 candidate programme theories have been developed in WP1 from key articles that are being further analysed using screened articles. MEMORABLE is using an innovative methodology, collaboratively, to develop an outcome-focused, evidence and experience-based framework to support and enhance practice and outcomes that are meaningful and relevant to those involved and the wider community.
