- W2896580122 abstract "There is a looming long-term care (LTC) crisis in the United States, and rural America will likely be hit the hardest. As a nation, the U.S. population is aging rapidly, as Baby Boomers have entered retirement, birth rates have been declining for several decades, and restrictive immigration policies are projected to slow the influx of younger people. This demographic imbalance is even more pronounced in rural areas, where 20% of the population is over 65, compared to 15% in the country as a whole (Baernholdt, Yan, Hinton, Rose, & Mattos, 2012). Compounding this problem is less access to LTC providers in rural settings. A potential solution to mitigate the undersupply of LTC in rural areas is to facilitate and increase the use of telemedicine and mobile health applications. Numerous studies have already demonstrated improved clinical outcomes through the use of telemedicine applications. For example, several studies assessing the impact of telemedicine monitoring of congestive heart failure patients reported a decrease in both mortality and hospital readmissions (Antonicelli et al., 2008; Clark, Inglis, McAlister, Cleland, & Stewart, 2007; Dang, Dimmick, & Kelkar, 2009; Kulshrestha, Kvedar, Goyal, Halpern, & Watson, 2010; O’Connor, et al., 2016; Polisena et al., 2010). In addition, four years of data gathered from the Veterans Health Administration national Care Coordination/Home Telehealth program demonstrated high patient satisfaction (85%), a 25% reduction of inpatient bed days, and a 19% reduction in hospital admissions (Darkins, 2013). This program addressed six chronic diseases, including diabetes, depression, and congestive heart failure. Regarding LTC in particular, the Veterans Health Administration estimated that this program facilitated the independent living of 36% of patients who otherwise were candidates for long-term residential care (Darkins, 2013)." @default.
