- W2896583136 abstract "To investigate whether cervical length measured by transperineal ultrasound is associated with time of delivery within three days. 2D ultrasound examinations were performed on nulliparous women with uncomplicated pregnancy within 12 hours of admission for show, irregular uterine contractions before onset of labour, in addition to digital vaginal examination on admission. Cervical length (CL) was measured by transperineal ultrasound and repeated up to two times where possible. Time of delivery and other demographic data were later retrieved from hospital's electronic database. Relationship between cervical length and time of delivery was analysed using Kaplan Meier survival curves, with delivery within 3 days set as primary end point. The accuracy of CL for the prediction of time of delivery within 3 days was analysed by means of the area under the receiver operating characteristic (ROC) curve (AUC). Of the 124 women recruited, 59 (47.6%) delivered within 3 days after ultrasound examination. 10 women whose labour was induced and delivered within 3 days were excluded from data analysis. Of the 114 women included, 49/114 (43.0%) delivered within 3 days. Cervical length was categorised into <1.5cm, 1.5-2.5cm and >2.5cm for Kaplan Meier survival analysis. 13/33 (39.4%) women with CL <1.5 cm; 29/53 (54.7%) with CL 1.5-2.5cm; and 7/28(25%) with CL >2.5cm delivered within 3 days. There was significant difference between the groups (log rank test P=0.032). The ROC curve of CL revealed a poor accuracy in the prediction of delivery within 3 days (AUC: 0.439; 95% C.I. 0.333-0.544). Cervical length measured by transperineal ultrasound before onset of labour was associated with delivery within 3 days, but there was poor predictive value.
