- W2896628816 abstract "The prevalence of dementia remains high and is predicted to rise as the population age distribution continues to shift to older ages. Interestingly, data from several large population-based epidemiological studies have at least demonstrated declining dementia incidence among more recent birth cohorts. This trend is thought to be related to a number of factors including increasing educational attainment and cognitive reserve, better cardiovascular risk factor control, and declining smoking habits. However, the relatively consistent cohort effect of declining dementia incidence rates has remained significant with adjustment for these and other exposure variables. We hypothesized a similar declining incidence rate would be observed in Western Pennsylvania in a pooled cohort sample and that part of this trend would be explained by education differences. We pooled data from two community-sampled prospective cohort studies with the same investigative team with similar study aims and sampling region: The Monongahela Valley Independent Elders Survey (MoVIES, 1987-2001) and The Monongahela-Youghiogheny Healthy Aging Team (MYHAT, 2006-Ongoing). Across both studies we identified four sufficiently sized decade-long birth cohorts spanning from 1902-1941. In an analysis sample of n=3010 participants (mean baseline age=75.7, 61% female, mean follow-up: 7.1 years), we identified 257 incident dementia cases diagnosed using the Clinical Dementia Rating. When compared to the most recent birth cohort (1932-1941), the oldest cohort (1902-1911) had a significantly higher incident dementia rate (HR=4.3, p<.001, 95% CI: 2.2 – 8.3) as did the second oldest cohort (1912-1921; HR=2.8, p=.001, 95% CI: 1.5 – 5.2). Analyses were adjusted for age, sex, and education. The cohort effect was consistent in sex-stratified analyses, with the additional finding that college educated women were less likely to develop dementia than women who did not finish high school (HR=.647, p=.039, 95% CI: .427 - .979). No education effect was observed in men. We observed a decline in incident dementia rates across subsequently younger birth cohorts independent of sex, education, and age. Further investigation remains to be done to examine factors influencing this well replicated trend, with education patterns likely playing a role." @default.
