- W2896636436 abstract "While a large body of evidence suggests that extreme stress such as PTSD can impact brain function and memory, less is known about the role of stress due to everyday problems, particularly in midlife, on dementia risk. We evaluated 12,263 Kaiser Permanente Northern California health plan members ages 40-55 in 1972-1973 who remained members as of 1/1/1996. In 1972-1973, participants self-reported “serious or disturbing problems” in the following domains (yes/no for each): marriage, family, drug usage, employment, finances, or “other”. Midlife stressors were examined individually, as any stressors (yes/no), and the number of stressors (count). Education, race, sex, and midlife health indicators (BMI, hypertension, and smoking) were also collected in 1972-1973. Dementia and late-life health indicators (stroke, heart failure, and diabetes) were abstracted from medical records from 1/1/1996-9/30/2017. Cox proportional hazard models (age as time scale) evaluated associations between midlife stressors and dementia adjusted for sociodemographics and lifecourse health indicators. Stratified models examined possible differences by race/ethnicity. Participants were censored at dementia diagnosis, death, gap in health-plan membership, or end of follow-up. Overall, 24% of the sample reported ≥1 midlife stressor (mean=0.4, SD=0.7); financial and employment problems 7% and 6%, respectively. Adjusted for sociodemographics, having any midlife stressors was associated with 15% greater risk of dementia (Table 1) and each additional stressor was associated with an 8% increased dementia risk. Financial and employment problems specifically increased dementia risk by approximately 25%. The associations between dementia and any stressor, financial stressors, and employment stressors persisted after adjusting for midlife and late-life health indicators. There was no evidence of an association between marital, familial, or drug problems and dementia. In stratified analyses, familial problems were associated with dementia risk among Blacks but not Whites or Hispanics. The association of employment problems with dementia was larger among Blacks compared to Whites (p-value=0.02 for interaction). Reporting serious problems in midlife was associated with elevated dementia risk with employment and financial problems increasing risk the most, particularly for Blacks. These findings suggest that problems related to sparse resources may impact brain health over the long-term and could be drivers of health inequities ." @default.
