- W2896645752 abstract "Type 2 diabetes (T2D) is associated with an approximately 2 fold increased risk of developing Alzheimer's Dementia (AD). Studies of brain glucose metabolism may help understanding of the mechanisms underlying this increased risk and may be valuable in detecting pre-clinical disease. We aimed to explore the associations of T2D with global and regional cerebral metabolism in a sample of middle-aged twins discordant for T2D without cognitive impairment. The sample was derived from the Australian Twin Registry, a national registry of volunteer twins in Australia. Twin pairs discordant for T2D were invited to participate if they were aged ≥50 years, with the diagnosis of T2D, and validated with fasting glucose levels <126 mg/dL (7.0mmol/L) confirming absence of T2D. All participants had 18Fludeoxyglucose (FDG) PET performed. We calculated an FDG Standardized Uptake Value Ratio (SUVR) by normalizing to the mean pons signal. Mean SUVR values were extracted from: whole brain, frontal lobe, parietal lobe, temporal lobe, limbic lobe, and occipital lobe. Linear mixed modelling was used to examine the association of T2D with brain FDG-PET to incorporate within-pair and between-pair variation. We recruited 23 (12 dizygotic pairs, 11 monozygotic pairs) pairs (mean age 63.7 years (SD 6.1)). The mean duration of T2D was 10.7 years (SD 11) and the mean pre-PET glucose in those with T2D was 7.9 (SD 1.5). None of the participants had dementia. T2D was associated with lower SUVR in whole brain, and lower bilateral frontal, parietal, temporal, occipital and cingulate cortices (all p<0.05). These associations remained even after adjusting for differences in sex and age between twin pairs. T2D was associated with cerebral hypometabolism in regions similar to those seen in AD. T2D may thus have adverse effect on neuronal function long before the development of clinically apparent cognitive decline. Mechanistic pathways involving glucose use and insulin resistance may be responsible for this, and require further study." @default.
