- W2896669144 abstract "Most cases of AD dementia are the result of mixed neuropathologies, but the degree to which AD pathology versus other neuropathologies contribute to the risk of AD dementia is not well established. We examined the risk of AD dementia attributable to standard pathologic indices of AD and eight other age-related neuropathologies. Data came from two clinical-pathologic studies. Participants underwent detailed clinical evaluations and autopsy; neuropathologic evaluations identified pathologic AD (NIA-Reagan criteria), macroscopic infarcts, microinfarcts, Lewy bodies, hippocampal sclerosis, TDP-43, cerebral amyloid angiopathy, atherosclerosis and arteriolosclerosis. Multivariable logistic regression models examined the associations of the neuropathologies with AD dementia and estimate the percentage of cases attributable to neuropathology. Considering that some AD dementia cases are driven by factors other than the measured neuropathologic indices, in subsequent analyses, we re-estimated the attributable risks after adjusting for empirically derived cases not driven by the neuropathologic indices. Out of a total of 1,161 persons, 512 (44.1%) had AD dementia at the time of death. With the exception of microinfarcts, all of the neuropathologies were independently associated with greater odds of AD dementia. In the initial analysis, a total of 212 (41.5%) AD dementia cases were attributable to pathologic AD. Separately, 9.3% were attributable to macroscopic infarcts, 10.8% to Lewy bodies, 5.3% to hippocampal sclerosis, 12.0% to TDP-43, 8.2% to CAA, 5.9% to atherosclerosis and 5.1% to arteriolosclerosis. Taking into account the co-occurrences of these pathologies, a total of 83.8% of AD dementia cases were attributable to these eight pathologies. However, in subsequent analysis adjusted for the derived 12% of AD dementia cases not driven by the neuropathologic indices, the percentage of AD dementia cases attributable to all eight pathologies dropped to 67.5%. Pathologic AD accounts for a considerable percentage of AD dementia cases, but other neuropathologies contribute independently to the risk of AD dementia.
