- W2896722824 abstract "Alzheimer's disease (AD) neuropathological subtypes [limbic predominant (LP), hippocampal sparing (HpSp) and typical (tAD)] have been proposed based on the absolute and relative neurofibrillary tangle burden in limbic and cortical brain regions. These subtypes have been studied in convenience samples with limited prospective clinical data. Here, we sought to characterize cognitive impairment in these subtypes in the Religious Orders Study and Memory and Aging Project, two prospective cohort studies with rich cognitive and neuropathological data. We hypothesized that the hippocampal sparing group would have faster cognitive decline, but relatively better memory performance. Among brain donors with an AD neuropathological diagnosis, with a Braak stage of 5 or 6 and without hippocampal sclerosis, we categorized participants into LP, HpSp and tAD subtypes using criteria from Murray et al. 2011. For each participant, we derived factor scores for memory, executive function, visuospatial ability and language at each study visit using structural equation models. In mixed effects models, we compared the rate of decline across subgroups for each cognitive domain and globally (domain average), using years before death as the time axis. Additionally, we regressed memory on the 3 other factor scores, and compared the residuals at the time of dementia diagnosis and on average across visits before and after dementia diagnosis. All models controlled for age at death, sex, and years of education. Among 1378 brain donors, 292 had an AD neuropathological diagnosis, a Braak stage of 5 or 6, no hippocampal sclerosis, and at least one visit with cognitive testing. There were 57 (20%) with LP, 22 (8%) with HpSp and 213 (73%) with tAD. For memory, language, and global scores, the HpSp group declined significantly faster, compared to tAD, while the LP group did not (Table 1). The HpSp group did not have relatively better memory performance prior to, at or after dementia diagnosis. The relative frequency and rate of cognitive decline of AD neuropathological subtypes in a population-based sample were similar to previous reports from a convenience sample. However, AD neuropathological subtypes may be incongruous with clinical AD subtypes defined by relative cognitive impairment." @default.
