• W2896735312 abstract "The risk factors that underlie sporadic early-onset Alzheimer's disease (sEOAD) are not well understood. We compared clinical presentation, demographic features, ApoE genotype, and dementia risk factors between patients with non-autosomal early-onset AD in patients with and without a family history of the disease. A group of 103 sEOAD patients with a ≤64 age of onset and confirmed evidence of amyloidosis using available PIB-PET or CSF values were included. Patients with known APP/PSEN1/PSEN2 mutations were excluded. Pedigrees were reviewed and participants were classified as: (1) no family history of AD/dementia in a first or second-degree relative, (2) dementia family history of AD/dementia in a first or second-degree relative. Group comparisons were performed across clinical and demographic variables (Table 1). Frequency of known AD risk factors and relevant comorbid conditions were also compared across groups (Figure 1A). Results. Patients with a family history of AD/dementia showed a lower frequency rate (27% vs. 46%) of atypical sEOAD (PCA, lvPPA) and a higher proportion of E4 Carriers (59% vs. 41%) compared to patients with no family history. Age-of-onset and clinical stage at first evaluation were similar. (Table 1).There were non-significant trends for patients without a family history to have higher rates of thyroid disease (a potential marker of auto-immunity at a young age) and TBI. No other risk factors differed across groups. Results were similar when dividing the family history group into first degree vs. second-degree relatives. When sub-classifying each group by ApoE4 genotype, in the no family history group there were higher rates of TBI in E4 carriers vs. non-carriers (Figure 1B). Results. A family history in patients with EOAD was associated with ApoE4 genotype, while absence of a family history was associated with a non-amnestic presentation. Traditional risk factors for late-onset AD do not explain non-familial EOAD, suggesting novel risk factors have yet to be identified." @default.
