- W2896768205 abstract "Combination: of rapidly accelerated fibrosarcoma kinase B (BRAF) inhibitor dabrafenib and mitogen-activated extracellular signal-regulated kinase (MEK) inhibitor trametinib are effective treatments for metastatic melanoma; however, their renal toxicity profiles are unclear. To date, only two reports concerning acute kidney injury (AKI) in patients treated with dabrafenib and trametinib exist. We report an 89-year-old woman with recurrent BRAF V600E-mutant melanoma who experienced reproducible AKI during dabrafenib/trametinib treatment that commenced after new pigmented lesions developed following first-line nivolumab treatment. Considering her older age and condition, she commenced treatment at one dose level below standard. Her baseline serum creatinine (Scr) and estimated glomerular filtration rate (eGFR) were 0.98 mg/dL and 40 mL/min/1.73 m2, respectively. After two weeks of treatment, her Scr rose to 1.31 mg/dL (eGFR, 29.4 mL/min/1.73 m2), but returned to 0.93 mg/dL when treatment re-stopped for two weeks. The regimen was then reintroduced at the same doses, but was interrupted three weeks later because her Scr level climbed to 1.28 mg/dL (eGFR of 30.2 mL/min/1.73 m2). After a second interruption and Scr drop, the regimen was reintroduced at lower doses that did not cause increased Scr. As of February 2018, her disease is still well controlled. To our knowledge, ours is the first patient reported to experience reproducible AKI associated with dabrafenib and trametinib. Our experience with this patient suggests that (1) kidney function should be monitored carefully with this combination therapy, (2) elevated Scr at an early treatment stage can decrease soon after dabrafenib interruption, and (3) AKI associated with dabrafenib and trametinib is likely to be dose-dependent." @default.
