- W2896769522 abstract "Disturbed sleep and altered circadian rest/activity rhythms (RARs) are prevalent in persons with clinical Alzheimer's disease (AD) but there are few data in preclinical AD. We compared sleep and circadian RARs between cognitively normal amyloid (+) participants in the A4 clinical trial, and cognitively normal amyloid (-) participants in its companion observational study, LEARN. A subset of participants from five A4/LEARN sites wore wrist actigraphs on their non-dominant wrists for seven consecutive 24-hour periods. We extracted standard sleep parameters, including total sleep time, wake after sleep onset, sleep efficiency, and sleep onset latency, averaged across nights. To characterize circadian RARs, we used raw activity count data to derive conventional metrics from the parametric cosinor model including mesor, amplitude, and acrophase and non-parametric metrics including interdaily stability, intradaily variability, and relative amplitude. We also applied function-on-scalar regression (FOSR), a novel data-driven approach free of parametric assumptions, to average and standard deviation 10-minute activity count profiles and adjusted FOSR models for age, sex, and education. p-values were not adjusted for multiple comparisons. Complete data were available from 34 LEARN and 26 A4 participants. They were broadly representative of the parent A4 and LEARN trials (mean age 71.2 ±4.6 years, 58% female, 16.6 ±2.4 years of education). Amyloid (-) and amyloid (+) participants did not differ by conventional sleep or parametric/non-parametric circadian metrics (Tables 1-3) . However, FOSR revealed that, compared to amyloid (-) participants, amyloid (+) participants had higher (p-value < 0.05) activity levels between 5:10 AM and 7:30 AM, and between 3:40 PM and 5:30 PM (Figure 1). Further, compared to amyloid (-) participants, amyloid (+) participants’ RARs showed greater (p-value < 0.05) variability across days between 4:40 AM and 8:10 AM, and greater stability between 3:40 PM and 5:20 PM." @default.
