- W2896797851 abstract "Imaging biomarkers of cerebrovascular disease (CVD) and neurodegeneration may potentially reflect different underlying processes related to Alzheimer's Disease (AD) pathology in late life. We sought to explore impacts of white matter hyperintensities (WMH, an index of CVD) and AD-signature cortical thickness on cognitive status in a community-dwelling older adult cohort from the Wake Forest Alzheimer's Disease Clinical Core (ADCC). N=130 ADCC participants (Table 1) underwent brain MRI (T1, FLAIR) and neuropsychological assessment. Participants were determined to be cognitively normal (CN) or have mild cognitive impairment (MCI) by consensus neuropsychological evaluation. Cortical thickness was calculated using FreeSurfer on T1; an “AD-signature” meta-ROI was calculated by averaging entorhinal, inferior and middle temporal, inferior parietal, fusiform, and precuneus regions (Schwarz et al., 2016). FLAIR were segmented using SPM Lesion Segmentation Tool; total WMH lesion volume was divided by FreeSurfer intracranial volume and log-normalized. Using a model-building approach, we constructed logistic regression models predicting cognitive status; variables included age, APOE-ɛ4 status, sex, WMH volume, and meta-ROI thickness. Predictors were retained if p< .10. WMH volume and meta-ROI thickness were significantly negatively related (r= -.429, p< .001, Figure 1a), as individuals with greater WMH volume had significantly lower meta-ROI cortical thickness; this was stronger in CN than in MCI (Figure 1b). A model including WMH and meta-ROI thickness significantly predicted cognitive status (adj. R2= .118, p= .0001); both WMH volume and meta-ROI thickness were significant predictors (Table 2). WMH volumes by cognitive status and median-split thickness are shown in Figure 2. Age, APOE-ɛ4 status and sex were not significant. All component regional model R2 values were lower than the meta-ROI model R2, with entorhinal cortex the best component region (R2= .117).
