- W2896840304 abstract "An important index of brain reserve is maximal attained brain size, which can be measured indirectly by head circumference. This study aimed to demonstrate the association between head circumference and risk of incident dementia in Chinese elderly living in the community. We conducted a prospective follow-up study (2014-2016) of 1,660 participants without dementia at baseline (2010-2011)in the Shanghai Aging Study. Socio-demographic, health behaviors, medical history, apolipoprotein E (APOE) genotype, and anthropometry of the participants were collected at baseline. Consensus diagnoses of incident dementia were conducted using DSM-IV criteria after the functional, neurological, and neuropsychological assessments. We identified 171 new-onset dementia cases (incidence rate: 2.00/100 person-years) with a mean of 5.2 years of follow-up. Cox regression models indicated that each 1 cm decrease in head circumference in male participants was associated with a 21% increasing risk of incident dementia (HR=1.21,95%CI:1.01-1.46). A 95% higher risk of incident dementia(HR=1.95,95%CI:1.01-3.75) was found among men with head circumference <=53 cm compared to those with head circumference >53 cm, adjusting for age, years of education, presence of an APOE-ε4 allele, obesity, cigarette smoking, heart disease, hypertension, diabetes and depression. Our results suggest that larger brain size may provide a greater reserve against cognitive decline in older Chinese men .
